PRISM STAR TRAINING
Accident & Incident Reporting
Clear records help care teams understand what happened, respond appropriately and prevent the same problems recurring. Build confidence in accident and incident reporting, from recording the facts and escalating concerns to using everyday incidents as opportunities for learning.
Clear records help care teams understand what happened, respond appropriately and prevent the same problems recurring. Build confidence in accident and incident reporting, from recording the facts and escalating concerns to using everyday incidents as opportunities for learning.
Work through the difference between observation, interpretation and opinion, and consider what makes a report useful to colleagues and managers. Reflect on how prompt reporting, clear follow-up and a supportive reporting culture can strengthen practice without turning the process into a blame exercise.

CARE PROVIDERS, FOR CARE PROVIDERS
Learning connected to everyday practice
Running care services means thinking about safety throughout the working day, for the people receiving support and the staff providing it. We bring that experience to learning that helps teams recognise risks, understand their responsibilities and relate the subject to everyday practice.
