Useful care records help colleagues understand the person, the support provided and what needs to happen next. Develop a clearer approach to report writing, focusing on factual language, relevant detail and information that supports continuity rather than creating unnecessary paperwork.

Work through the difference between observation and interpretation, and consider how tone and wording affect the person described.
