PRISM STAR TRAINING
Writing Care Plans
Care plans should help staff understand the person and provide consistent, individual support. Develop a clearer approach to writing plans that describe priorities, preferences and practical actions, giving teams useful information they can apply during everyday care.
Care plans should help staff understand the person and provide consistent, individual support. Develop a clearer approach to writing plans that describe priorities, preferences and practical actions, giving teams useful information they can apply during everyday care.
Explore the difference between specific, person-centred wording and generic statements that tell staff little about what to do. Practical examples consider involvement, respectful language and review, helping plans remain relevant to the person's choices and changing needs rather than becoming static documents.
